South East TorontoStroke Network (SETSN)

    The SETSN is one of 11 regional networks established across the province to implement the Ontario Stroke System. The primary objective is to support innovation, knowledge translation and practice improvements across the South East Toronto Region, with the goal of promoting equitable access and improved outcomes for stroke survivors and their families. This Network represents a collaboration of regional partners in stroke care delivery that spans the areas of prevention, health promotion, acute care, rehabilitation, long term care facilities and community providers. Strengthening of these partnerships through the SETSN will lead to reorganization of stroke care, system change, professional education on stroke best practices and enhanced public awareness and resources.
    St Michael's Hospital is a designated Regional Stroke Centre providing investigative and treatment services through emergency and acute care services. A Stroke Assessment and Treatment Team within the hospital is available for assessment and rehabilitation services.
    Secondary Stroke Prevention services are also available within St Michael's.
    The Regional Network Team consists of Program Manager, Regional Education Coordinator, Regional Clinical Nurse Specialist, Rehab and Community Reengagement Coordinator and Program Assistant. This team supports regional partners through advocacy, leadership, governance, data management, transition management, awareness, education and resources, related to stroke.
    Region spans Yonge St to Rouge River, Eglinton to the Lake

West GTA Stroke Network

Trillium is an active member of the West GTA Stroke Network, a consortium of hospitals and Community Care Access Centres working with the Heart and Stroke Foundation of Ontario, the Ministry of Health and Long-Term Care and District Health Councils to implement a coordinated approach to stroke care through the West GTA.

The vision of the West GTA Stroke Network is that stroke outcomes can be dramatically improved and quality of life enhanced through awareness, education, and prevention measures. The Network is committed to ensuring all individuals have timely access to effective, client- and caregiver-friendly, seamless stroke care.
