About The Program

This year, the Prevent Stroke team will visit several Ontario cities to facilitate 'Connecting the Dots' workshops. So far, we have helped plan Connecting the Dots events in Owen Sound, Thunder Bay, Windsor-Essex, Grey-Bruce and Hastings-Prince Edward County, Chatham-Kent, Northwestern Ontario, Huron Perth, Central East, Peel Region, Stratford, Ottawa and Toronto's Trillium Health Centre.

What is Connecting the Dots?

It's a dynamic community process that sets the stage for collaborative work on chronic disease prevention. Connecting the Dots brings together a wide range of people working in different sectors to increase awareness and opportunities for collaboration. It helps people talk about how to 'work together differently' in their communities throughout the continuum of chronic disease, including health promotion, public health, hospitals, community services, and long-term care.
